Rahma Mohamed is the Owner of Care2TheNW and a Geriatric Practitioner, Social Worker, Complex Illness Navigator, and Certified CPR & First Aid Instructor. With a lifelong passion for serving seniors and supporting care providers, she brings extensive experience guiding families, seniors, and care facilities through complex care decisions, regulatory compliance, and safety training—with compassion, expertise, and integrity.
Rahma’s commitment to senior care began in her teenage years while working as a wait staff member in an assisted living facility. During that time, she developed a deep respect for older adults and gained firsthand insight into the importance of dignity, safety, and quality care. These early experiences shaped her career path and inspired her dedication to advocating for seniors and their families.
Over the years, Rahma advanced her education and professional training, becoming a Social Worker and Complex Illness Navigator. Her hands-on experience expanded further through owning and operating an assisted living facility, giving her a unique, real-world understanding of both the clinical and operational challenges care providers face. This perspective allows her to support organizations not only with compliance, but with practical, sustainable solutions that truly work.
Rahma specializes in senior care placement, care planning, and compliance support for assisted living facilities, group homes, non-emergency transportation providers, and daycare programs. She is widely respected for her ability to help organizations resolve citations, strengthen care plans, and maintain regulatory compliance while ensuring high-quality, client-centered care.
In addition to direct services, Rahma is deeply committed to education. She provides training, guidance, and resources that empower families to make informed care decisions and help care providers implement best practices that promote safety, accountability, and excellence in care.
